New Patients
The safety and well-being of our patients and staff is always of utmost importance.  For the health and safety of our patients, staff, and community, we have increased our efforts to help contain the progression of the COVID-19 pandemic. Following the guidelines of the American Academy of Dermatology and Governor DeSantis, we are postponing all upcoming appointments. We will be available to see urgent skin conditions such as severe rashes or post-operative concerns, and plan to continue treatment of high risk skin cancers. We are hoping to offer teledermatology services in the near future for follow-ups for patients with acne, rosacea, eczema and psoriasis.

Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

New Patients 

Your first visit to Sand Lake Dermatology Center involves a few special steps. Here are some helpful hints for an easy transition to our office: 

  • Please fill out the set of New Patient Forms provided above and bring them with you to your first appointment. 
  • If you have any medical reports that you would like us to review that were performed at another office, please obtain them prior to your appointment and bring them with you. 
  • Don't forget to bring your medical insurance card and driver's license to your appointment. 
  • Dependents under the age of 18 years old should be accompanied by a legal guardian for their first office visit. 

Established Patients

Your medical records from our office can be easily accessed at any time by simply visiting These records include office visit notes. Each patient is assigned a unique username and password to log on to our online patient portal. If you are interested in accessing your medical records online, please call 407-352-8553 and ask for your patient portal log-in information.

If you would like a hard copy of your medical records or would like your records to be sent to another physician, please complete the Medical Records Release form and fax it to our office at (407) 351-8412
