Patient Forms
Patients can fill out patient forms online through a personal patient portal. If you haven't received your patient portal link, please reach out to our office to have the patient portal link resent so you can update your medical history and other information online. If a patient does not have the ability to use our patient portal we can certainly email them the Patient forms via PDF that can be completed in advance of their appointment.
New Patients
Your first visit to Sand Lake Dermatology Center involves a few special steps. Here are some helpful hints for an easy transition to our office:
- Please fill out the set of New Patient Forms in your patient portal. If you haven't received your patient portal link, please reach out to our office to have the patient portal link resent so you can update your medical history and other information online.
- If you have any medical reports that you would like us to review that were performed at another office, please obtain them prior to your appointment and bring them with you.
- Don't forget to bring your medical insurance card and driver's license to your appointment.
- Dependents under the age of 18 years old should be accompanied by a legal guardian for their first office visit.
Established Patients
Your medical records from our office can be easily accessed at any time by simply visiting sandlake.ema.md. These records include office visit notes. Each patient is assigned a unique username and password to log on to our online patient portal. If you are interested in accessing your medical records online, please call 407-352-8553 and ask for your patient portal log-in information.
If you would like a hard copy of your medical records or would like your records to be sent to another physician, please complete the Medical Records Release form and fax it to our office at (407) 351-8412.
